Birth Registration and New Passport for Child

1. Submit original passports of parents
2. Submit original and a copy of Birth Certificate issued by Ministry of Health duly attested by Ministry of Foreign Affairs.
3. Submit prescribed application for information regarding parents & Child (Application available in the Embassy - Free of charge)
4. Submit application form for miscellaneous services
5. Paste one recent photograph of mother (35 x 45 mm) on application and sign across with half the signature on the photograph and half on the application.
6. Attach 2 recent photographs of child
7. Husband's name must be endorsed in wife's passport before registration of child birth.

5. Paste one recent photograph of mother (35 x 45 mm) on application and sign across with half the signature on the photograph and half on the application.
6. Submit new passport application form for New Passport EA (P) 1
7. Paste one recent photograph of child (35 x 45 mm) on application and paste child thumb impression across with half the impression on the photograph and half on the application.
8. Attach 2 recent photographs of child
9. Husband's name must be endorsed in wife's passport before birth registration.

Change of Name and Endorsement of Husband's Name After Marriage - KD 9.600

1. Submit both (husband's and wife's) original passports
2. Submit application form for miscellaneous services EA(P) 2
3. Paste one recent photograph of wife (35x45 mm) on application and sign across with half the signature on the photograph and half on the application.
4. Submit prescribed deed for the change of name after marriage and/or addition of husband's name in her passport (Deed form available in the Embassy - Free of charge )
5. Submit marriage affidavit in English only signed by the wife (Affidavit available in the Embassy - Free of Charge)
6. Submit original and copy of marriage certificate
7. Personal presence of wife in the Embassy is essential.

Change of Name (MINOR) - Fee : KD. 6.350

1. Submit application form for miscellaneous services EA (P) 2
2. Paste one recent photograph of child (35 x 45 mm) on application and sign across ( or paste child thumb impression ) with half the signature on the photograph and half on the application.
3. Submit sworn affidavit for change of name of a minor signed by father or mother
4. Submit original passport.
5. Those born in Kuwait should first change their name on the
   birth card issued by Ministry of Health prior to change of name on
   passport. For the same a no-objection certificate can be obtained
   from the Embassy for obtaining a new birth card with new name from
   the Ministry of Health. (Fee for no-objection certificate is KD 6.000)

   Note: Applicant should publish name change in daily news paper (one in Kuwait and one in India in any daily news paper of his/her domicile)
